To schedule an appointment, all requests must be sent through the form on this page, by fax, or by email to [email protected]
Please ensure your name, address, and phone number are clearly written on your request form.
Attach any relevant radiology reports, including axial CT scans or MRI results. These must be dated within the past five years.
Kindly note that results provided on CD are not sufficient to book an appointment.
Depending on several factors, wait times may exceed six months. Thank you for your understanding.
